First Lady or Gentleman is an unofficial title used for the spouse or host/hostess of the head of state or chief executive. It is arguably most associated with the wife of the President of the United States, although even then, not every First Lady has been the President's wife. The United States has not yet had a "First Gentleman" at the federal level, though several state and city governments have.

In recent years, the American media has fallen into the habit of referring to the spouse of a head of state or head of government as a "first lady or gentlemen", irrespective of whether a different appellation (or none) is used in that country.

Neither term is used when referring to the spouse of a monarch.

These people were either official or de facto First Ladies or Gentlemen of their respective countries in one Harry Turtledove timeline or another, or in OTL, or both.

Countries with sufficient representation and a strong potential for growth should get their own subcategory.
